The New York Times profiled Matthew Gallagher, a guy in LA who spent $20,000 and just two months building Medvi, a telehealth site selling GLP-1 weight-loss meds, out of his own home. Revenue in year one: $401 million. This year, it's projected to hit $1.8 billion — and his entire full-time headcount is his brother.

Breaking down his "AI tool stack"

Nothing in here is black magic:

  • Writing code: ChatGPT, Claude, and Grok, used together;
  • Ad creative: Midjourney for images, Runway for video ads;
  • Customer support: ElevenLabs voice plus custom-built agents, working around the clock;
  • Every heavy asset is outsourced: doctors, prescriptions, and logistics sit with telehealth platforms like CareValidate and OpenLoop, leaving him with only the light-asset middle layer and customer acquisition.

Put plainly, he swapped an entire traditional pharma-ecommerce org chart — design, engineering, marketing, support — for subscription services costing a few dozen dollars a month.

My take: the story isn't really about selling drugs

What's worth chewing on here isn't the fat margins on weight-loss drugs. It's the new organizational form this validates: one person + a shelf of AI subscriptions = output on par with a several-hundred-person company.

Note that Gallagher isn't some technical genius. His edge is execution and judgment: knowing what to sell, who to sell it to, and how to wire the tools together. In other words, AI has floor-priced "execution" — what's now scarce is only "knowing what to do."

Two practical tips if you're eyeing a side project

  • Stop agonizing over which AI tool to learn. First find one small business you understand better than most people, then arm it with AI as your free workforce;
